How do health insurance exchanges work?

The health insurance exchanges are simply individual insurance plans that are offering their services through the state run programs.  These companies have placed bids and have been selected by the state exchanges.  Most people will still be able to purchase individual insurance through insurance companies that have chosen not to offer their plans through the state.
